Introduction: Schwannoma arises from the Schwann cells of the sheath of myelinated nerves. Only 4% involve the sinonasal tract; ethmoid followed by the maxillary sinus are the most frequently affected.
Case description: We report a rare case of ancient schwannoma in a 75-year-old male, arising from the anterior nasal septum. Histopathology after complete excision of the mass confirmed the diagnosis of ancient schwannoma. There has been no evidence of disease recurrence after 2 years of follow-up.
Discussion: It is difficult to determine the neural origin in nasal schwannomas, but suggested origin sources include the sympathetic nerves, the parasympathetic nerves, and the sensory nerves to the septum. Common symptoms include nasal obstruction, epistaxis, anosmia, and nasal deformity. These tumors can cause significant morbidity if left untreated. Microscopic features include Antoni A and Antoni B areas, and the characteristic feature of the palisading cell arrangement called Verocay bodies. Ancient schwannomas exhibit clear areas of hypocellularity and hyalinization, and these changes are attributed to long-standing degenerative changes.
Conclusion: Schwannomas arising from the nasal septum are exceedingly rare, but should be considered in the differential diagnosis in patients presenting with nasal septal masses. These tumors are typically benign and have a very low recurrence rate postexcision. Treatment requires surgical excision with close follow-up.
